Inquest finds multi-agency failures before the death of Jessie Laverack

Coroner to make a report to prevent future deaths addressed to the Home Secretary, the Minister for Health and the Minister for Justice for better training and recognition of the link between domestic abuse and suicide.

Jessie Laverack was a high risk victim of domestic abuse with a complex array of support needs. The Coroner made a series of critical findings and concluded that agencies failed to properly coordinate Jessie’s support and that therefore her risk of suicide in the context of domestic abuse was not properly understood or assessed.
