Inquest jury find that a systemic failure’s by police preceded Donna Williamson domestic homicide

Donna died from stab wounds inflicted upon her by her ex-partner Kevin O’Regan.

Reaching a conclusion of unlawful killing, the jury concluded in a damning narrative that the police had failed to manage the fatal risk posed to Donna by Kevin O’Regan.
