Inquest finds that Donna was living in hazardous conditions prior to her death in 2018 and that serious failings on the part of Donna’s husband, the London Borough of Newham, and East London NHS Foundation Trust contributed to her death.
The Coroner also indicated her intention to make a report to prevent future deaths around the failure by the Trust to document, assess and manage the risk around Donna’s ingestion of unprescribed medication.
