A jury has returned a damning narrative concluding that serious failings on the part of London Borough of Islington, London Borough of Ealing and the West London NHS Trust contributed to Lance’s death whilst living in unregulated accommodation.
The Coroner also indicated her intention to make a report to prevent future death in relation to unregulated accommodation for care leavers.
